Have you ever FAILED a grade OR got a D/50% in anything?

Yes I've failed on my physics exam. I was so sick, i could barely sit upright and answer the paper. Thank god ! They let me take it again, would have lost a whole year otherwise.

Actually I've never ever failed in my grades in my whole life of existence. But there are times when I get low grades. But the thing is as long as I don't fail getting low grades is okay for me. That's because I don't really study much and exert too much effort. I am happy go lucky most of the times. But when I study and get low grades that would be very DEPRESSING.
 
No, never. The lowest I have ever gotten was a C.

Oh what a nice question... but in my entitre school year I haven't been failed.
