I have no idea about it on Xbox One, but on 360 if you have a Diamond Horizon account and a USB you can mod the gamerscore for games.

I have an xbox account with 50k+ gamerscore and 5 year tenure that has never been banned since I have done it.

I did it in like 2013 when I thought Horizon was cool. So no, within the past 4 years. I know I said 2013 but I really don't remember.
