Second I would like to correct you with a point. The request form is basically a form they should post with then doing a thread, not requesting a review copy.

I don't understand. The full form posted there is for a reviewer to post with info to the seller if they wish to have there Ebook reviewed.
The second is for the user to use to send to a Leader to determine info on the Ebook and send it to a reviewer.
