Who owns ___ Gamertag?

I've noticed many members posting individual threads in order to find out who owns a Gamertag. Instead of doing that, I'm going to create this thread and include a post template below which will allow you to post the Gamertag to find out more information on it. Information on the Gamertag may include when it was reset, the Gamertag's tenure, and the Gamertag/account's owner.
