It'd take a small amount of PHP also, not just HTML. I can maybe design one tomorrow and go advanced into it, with little checks and such, but Images will need to be upload, unless done externally.

A little PHP? you would need quite a bit actually, and the paypal button would need to direct them to a URL after they purchase it which could change their usergroup (I believe there is a pre-made script that does that)
