Recovering an Xbox account question.

I have a long lost xbox account, I had one earlier that was hacked and I was abke to recover it by filling out a form with personal info. Does anyone know how to get to that form, because the support I have talked to are just fucking ridiculous at this point.
 
Go log in to the email like you normally would, that's attached to the Xbox account. Click can't access my account, then click "I forgot my password", or "I think someone else is using my Microsoft account". Keep going through those pages until you come to a page where you can fill out any information regarding the account.

someone told me I was hacked.
Put the email you need.
I no long have access to these and fill out the rest.
Hopefully this is what you meant by form.
 
@FeaR no I no longer know the email at all. Last time I wejt through it without knowing the email at all the let me fill out a form through the phone with past console ids. First and last name on account address, security quest and answer, and so forth. Now they are acting like dumb asses.
 
That is something they send out which is called a UA. 
You will not just find this form any where.

What can I say to request one?
I'm looking to get it back as I havent played in 3 years
