How many people Mod as in Design them.?
How many just use them?

I'm into abit of both and doing some Fallout NV and TES Oblivion mod creating.
 
I have never tried it, but me and my mates had an idea to start making own game on some game motors.
We'll start with Doom 1 and maybe will make one on Half life's, at least that's what we're planning.

If your running it from a direct engine.
You'll need to be able to code in C++
Also check the policy of changing their code.
You don't want to face any legal battles.
