I'm thinking of buying a $20 PSN card to buy CoD 4 (first MW). But I'm not sure.
Do you think that buying CoD 4 is a good idea? Or should I buy another game with the $20? (No, not BF3 because it is like $60)

Well, it depends on how you view the Call of Duty series.

I personally hate all CoD games except for CoD4. If you don't mind all the modded lobbies, then I'd say yes it's worth it. But then again, I'm on 360 so I don't know how bad all the modding is for PS3.
